7. DMV Statements
8. Closing statements pertaining to real estate
transactions.
9. Any tax notices received from the IRS or other taxing
authorities

Does any of children under 14 have interest or dividend income in excess of

Did you receive unreported tip income of $20 or more in any month?
Did you cash any Series EE U.S. savings bonds issued after 1989 an pay qualified
higher education for yourself, spouse, dependents?
Did you receive a distribution from a profit and sharing plan, retirement
plan, or IRA. (including ROTH and Traditional)?
Did you receive disability income?
Did you hand any foreign income or pay any foreign taxes?
Did you start your own business / farm?
Did you purchase rental property?
Acquire interest in a Partnership?
Did you purchase or dispose of business asset?
Did you buy or sell any stocks?
Did you purchase, sell, or refinance your principal home or second
home?
Did you have any debt forgiven?
Did anyone owe you money which had become uncollectable?
Did you incur a loss because of damaged or stolen property?
Did you work out of town for part of the year?
